Why is Accountability So Hard?

I have been thinking a lot about why accountability is so hard in some team cultures.

Why do we avoid accountability? Why do we avoid holding others accountable? Why don’t we have the hard conversation when we first see something that isn’t quite what the team agreed upon? Why do we ignore known issues until they become catastrophic and can no longer be ignored? Why do we tend to vilify those who are charged with holding us accountable?
