Too many rules are a sign of a controlling environment. Controlled is not necessarily safe. Rules made out of a reaction to an embarrassing failure tend to be restrictive and rescinded when the emotion of the failure fades away. Rules should be made when we are calmly assessing a situation and want to learn from our past failures. Rules are not in place to control people to ensure failure is not an option. When that happens, an unsafe environment is developed where people fear failure because another restrictive rule is expected.
